Documents for Admission

 Ukrainian Program (based on NMT / EVI / EFVV results)

To enroll in the Ukrainian program, the following documents must be submitted:

  1. Passport (original and copy).
    *If submitting a passport with a contactless electronic chip, a copy of the certificate/extract of residence registration must be included.
  2. Identification number (original and copy).
  3. Document of previous education on the basis of which admission is made, or any foreign document of previous education, if NMT 2025 results are available (original and copy).
  4. Transcript with grades accompanying the previous education document (original and copy of all pages).
  5. NMT certificate (for bachelor's admission) or examination sheet of the unified entrance exam / unified professional entrance test (for master's admission).
  6. Printed NMT / EVI / EFVV results on blank A4 sheets.
  7. Certificate of name change (if applicable).
  8. Four color photographs (3×4 cm).
  9. Military ID (from age 25) or temporary certificate (from age 25), or conscription registration certificate (under age 25) (original and copy), or an extract from the "Reserve+" mobile application.
  10. Extract from the order or copy of the military unit commander's order granting permission to study without interruption of service (for active military personnel).
  11. Motivation letter printed on a blank A4 sheet.

  Ukrainian-Polish Program

To enroll in the Polish-Ukrainian program, ORIGINALS, COPIES, and SCANNED COPIES of the following documents must be submitted:

  1. International passport of a citizen of Ukraine (original, quality copy).
  2. Passport of a citizen of Ukraine (original, quality copy).
    *If submitting a passport with a contactless electronic chip, a copy of the certificate/extract of residence registration must be included.
  3. Identification code (original, quality copy).
  4. Education document on the basis of which admission is made (original, quality copy).
  5. APOSTILLE of the education document on the basis of which admission is made (must be obtained first!!! before submitting documents for translation into Polish).
  6. NOTARIZED TRANSLATION INTO POLISH of the education document on the basis of which admission is made (translation is done only after the apostille is affixed!!!).
  7. Transcript with grades accompanying the education document (original, quality copy of all pages).
  8. APOSTILLE of the transcript accompanying the education document (must be obtained first!!! before submitting documents for translation into Polish).
  9. NOTARIZED TRANSLATION INTO POLISH of the transcript accompanying the education document (translation is done only after the apostille is affixed!!!).
  10. Certificate of name change, if applicable (original, quality copy).
  11. NOTARIZED TRANSLATION INTO POLISH of the name change certificate (original).
  12. B2-level certificate of Polish language proficiency (if necessary, Polish language courses can be taken and a certificate obtained at a Polish higher education institution).
  13. Four color photographs, 3×4 cm.

ATTENTION! For master's admission in LAW and PSYCHOLOGY specialties, two previous education documents are required: bachelor's diploma and secondary school certificate, along with their apostille and notarized translation into Polish.
