Admission Documents
Ukrainian Program (based on NMT / EVI / EFVV results)
To apply for studies under the Ukrainian program, the following documents must be submitted:

Passport.
*In case of submitting a passport with a contactless electronic chip, a copy of the certificate / extract of registration of the place of residence must be attached. -
Identification number.

A document on previous education on the basis of which admission is carried out, or any foreign document on previous education, provided there are results of national graduation exams in 2025-2026 after high school in Europe.

NMT Certificate with an information card (for admission to bachelor's programs).
-
Examination sheet of the Unified Entrance Exam / Unified Professional Entrance Examination (for admission to master's programs).

Printed results of NMT / EVI / EFVV on clean sheets of A4 format.

Certificate of change of surname, first name, or patronymic (if necessary).

Four colored photographs (3x4 cm).

Military ID (from 25 years old) or temporary certificate (from 25 years old), or registration certificate to draft registration centers (under 25 years old), or an extract from the mobile application "Reserve+".

An extract from the order or a copy of the order of the military unit commander granting permission to study full-time while serving (for active duty military personnel).
-
Combatant certificate (if available).

A certificate confirming the presence of a preferential category (see the section "Applicants", subsection "Special conditions of admission (preferential categories)")
Ukrainian-Polish Program
To apply for studies under the Polish-Ukrainian program, it is necessary to submit ORIGINALS, COPIES, and SCAN COPIES of the following documents:

International passport of a citizen of Ukraine.

Passport of a citizen of Ukraine.
*In case of submitting a passport with a contactless electronic chip, a copy of the certificate / extract of registration of the place of residence must be attached. -
Identification code.

An education document on the basis of which admission takes place (complete general secondary education (11 grades).

An transcript with grades attached to the education document on the basis of which admission takes place (complete general secondary education (11 grades).

APOSTILLE of the document on obtaining complete general secondary education (11 grades) (must be done as a priority!!!
before submitting documents for translation into Polish). -
NOTARIZED TRANSLATION INTO POLISH of the document on obtaining complete general secondary education (11 grades) (translation of the education document is carried out exclusively if there is an apostille on it!!!).

APOSTILLE of the transcript with grades attached to the document on obtaining complete general secondary education (11 grades) (must be done as a priority!!!
before submitting documents for translation into Polish). -
NOTARIZED TRANSLATION INTO POLISH of the transcript attached to the document on obtaining complete general secondary education (11 grades) (translation of the transcript to the education document is carried out exclusively if there is an apostille on it!!!).

Certificate of change of surname, first name, or patronymic if necessary.

NOTARIZED TRANSLATION INTO POLISH of the certificate of change of surname / first name (original).

Four colored photographs measuring 3x4 cm.
Ukrainian-Kazakhstani Program
To apply for studies under the Ukrainian-Kazakhstani program, the following documents must be submitted:
- International passport of a citizen of Ukraine (original, high-quality copy).
- Passport of a citizen of Ukraine (original, high-quality copy).
*In case of submitting a passport with a contactless electronic chip, a copy of the certificate / extract of registration of the place of residence must be attached. - Identification code (original, high-quality copy).
- An education document on the basis of which admission takes place (original, high-quality copy).
- A transcript with grades attached to the education document on the basis of which admission takes place (original, high-quality copy of all pages).
- TRANSLATION INTO KAZAKH of education documents (diploma and transcript) on the basis of which admission takes place.
- Certificate of change of surname, first name, or patronymic if necessary (original, high-quality copy).
- TRANSLATION INTO KAZAKH of the certificate of change of surname / first name (original).
- Four colored photographs measuring 3x4 cm.
