Entry Documents
For admission to study under the Ukrainian program, the following documents must be submitted:
- Passport (original and copy).
*In case of submitting a passport with a contactless electronic chip, it is necessary to add a copy of the certificate/extract on registration of residence. - Identification number (original and copy).
- Document of previous education on the basis of which the admission is made, or any foreign document of previous education, if there are NMT 2025 results (original and copy).
- Supplement with grades to the document of previous education (original and copy of all pages).
- NMT certificate (for admission to bachelor's degree) or examination sheet of the unified entrance exam / unified professional entrance test (for admission to master's degree).
- Printed results of NMT / UEE / UPET on blank A4 sheets.
- Certificate of change of surname, first name or patronymic (if necessary).
- Four color photographs (3x4 cm).
- Military ID (from 25 years old) or temporary certificate (from 25 years old), or certificate of registration at conscription stations (under 25 years old) (original and copy), or extract from the mobile application "Reserve+".
- Extract from the order or copy of the order of the commander of the military unit on granting permission to study without interruption from service (for active military personnel).
- Motivation letter, printed on a blank A4 sheet.

For admission to study under the Polish-Ukrainian program, it is necessary to submit ORIGINALS, COPIES and SCANNED COPIES of documents:
- Passport of a citizen of Ukraine (original, copy, scanned copy).
*In case of submitting a passport with a contactless electronic chip, it is necessary to add a copy and scan of the certificate/extract on registration of residence. - Identification code (original, copy and scanned copy).
- Document of education on the basis of which the admission is made (original, copy and scanned copy).
- Supplement to the document of education on the basis of which the admission is made (original, copy and scanned copy of all pages).
